Why is my BOM issuing 3 times the number of units for a component than the BOM calls for

I have a Finished Good with a Primary UOM of Square Inch and secondary in EA and a Manufacturing in EA

The component has Primary UOM in Square In and a Secondary in LB

There are appropriate UOM Conversions at the item level and they check out.  This is a consistent process across all Finished Goods

All other items are set in the same manner, and they all perform correctly.  Only this item is a problem.

When I run R31410 to attach the parts list, the parts list is attached with 3 times the number of component items as the required amount, unless I create the work order for just one EA.
