Cardex vs Work Order

We are having issues with the Cardex against the actual transactions that happen.       

Work Order 123 is showing we issued part number 1   for 100 pieces

The cardex is showing that the 100 pcs were issued twice by different users sometimes 1/2 hr apart.  the second one, the user never went into that work order.   the work order is only showing the 100 issued, the cardex is showing 200 issued.

Anyone ever see this before?
