Same S/N with different items

Hi Team,

Please assist us to prevent the below scenario where in IB system allowed us to create 2 different item instances with same S/N, ideally system should stop us while we tried to create the item instance with same S/N.

Please advice why it didnt happen and system created 2 instances against the same S/N. Please see the below screen shot items are different.

Need very urgent assistance.
