Upgrade E-Business Suite from R12.1.1 to R12.2.0

Hi,

We are running our EBS with two node, where DB version - 11.1.0.7 and EBS version - R12.1.1. We are now willing to upgrade EBS with DB to 12c and Apps R12.2.0. Is there a certain doc or white paper I can rely on to work on upgrade.

Much appreciated, If I am provided with Action Plan as well.
