Debit Memo not raised

This happens randomly for a few RTV transactions in which the RTV completes but the Debit Memo is not getting raised. I have checked all required setups and they are ok. Even i tested for same PO and line RTV for another receipt for that DM is raised . But sometimes for some GRR the Debit Memo does not get raised.

My query is :

1. If all setups are ok then what can be reason for DM not getting raised.

2. Once DM is not raised after RTV completes. Can we run some program by which the DM can be created.
