Duplicate Charges lines

Hello Team,

Not sure what happen with few SRs we created against those the 'ACTUAL' charges lines manually added and after we submit, the actual lines gets copied automatically and we can see the double amount of the charges submitted. For e.g if there are 5 lines we submit then it gets 10 with submitted status.

Unfortunately I am not able to replicate this scenario i my tests  also user is confident that he haven't create the dup. lines by  himself. I have attached the screen shots from the charges tab please review and advice what is this issue and how it can be possible?
