Duplicate Files Uploaded

Hi Group, I'm hoping someone might be able to help me. My company is using Agile v. 9.3.3. Intermittently the system is loading duplicate files and skipping some files when adding a group of files at the same time. This is happening when dragging and dropping the files into the File Upload Selector window, or by selecting "Browse for files," selecting the group of files, and then clicking "Open." It is happening with Chrome and IE web browsers.

For example, the user uploads these files:

SOP-10001.docx
SOP-10001 Attach 1.docx
SOP-10001 Attach 2.docx

But what gets uploaded is this:

SOP-10001.docx
