Outcome_id not populating correctly (interaction wrap-up)

When I click on the green light (start interactions) and enter the outcome, results and reason and then click end (wrap-up) interactions.    it will create in the records in the activities table, but the outcome_id code is showing 107 the default instead of 114.  see images below.

When I start the interactions, the Default values for Action and Action Item and top field Outcome is setup automatically.

If I tab in the Action and Action Item in the bottom fields and then enter the outcome, results and reasons then it will use the correct outcome_id 114.

is there something I missing, I check the profiles and everything looks good see images below?  do I have to tab on the action and action item to populate the correct outcome_id?  I never did that before.  this is happening in all environments.
