Two errors 1094C-028 and 1095C-010-01

Two errors 1094C-028 and 1095C-010-01 We have an employee whose name is not corrrect with the IRS which is why were are getting 1095C-010-01.  Could that be the reason that we are receiving 1094C-028?  Details say Total number of Forms 1095-C filed by and/or on behalf of ALE Member is missing 1094C-028.    Any feedback will be helpful!
