Duplicate journal lines created in GL for cost management transactions

Cost management has created duplicate entries in GL for majority of the transactions in past period. It was only detected after create accounting was run.

Below is an example of transaction in inventory and its accounting.

We need to find out why this has occurred, how to rectify this bug, and how to adjust GL balances.
