Creating notes using Tools menu. Form disappears behind SR form when saving Note.

We have recently applied patches to upgrade to 12.2.5 which has updated the CSXSRISR form. We have a problem when using the Tools menu to create new SR notes. Upon saving the Note the notes form flashes up twice and then disappears behind the SR form. It is as though the cursor is returning to the SR screen rather than remaining on the Notes form. We have tried switching off forms personalisations, custom code and the folder but this still happens. Has anyone else seen this happen? Is there a solution to prevent this?
