Purchase order waiting for approval not appearing in orders awaiting approval

I have seen a couple of purchase orders at status 230-230  that don't appear in P43081. In one case the originator was fourth highest on the approval route in another case the originator wasn't in the approval route. Both on JDE9.2. The most recent one (that hasn't been cancelled yet) is sitting in F4209 and F4301. Has anyone experienced the same and what caused/solved it?
