Would a failed boot disk raise an ASR?

A field engineer while performing dimm replacement discovered that there was a failed boot disk on node0?  An ASR was not raised for this issue which makes us wonder if one should have been raised, or ASR is not properly setup.  Has anyone experience the same issue?
