Form applet messed up with mismatching labels and fields

Some users experience sporadic shifting of information that’s been already populated between fields. In these cases, the labels on the applet is in order but the fields do not belong to that label. Example a Date field will have a number and the Name field will have the date.  Leaving the View and returning to it appeared to resolve the problem. Clearing the cache did not resolve the issue. The user have experienced this issue more often when they close a validation pop-up applet on the view. I have tried to reproduce it using the same steps and I cannot
