E-pro requisition attachments

User has the following problem:

I encounter this issue at least a couple times a month that I attach my documents, do the rest of the E-Pro requisition, send it on and somewhere in the process, the attachment falls off.  On this particular document, I think it fell off twice, (not certain since it started the routing almost a month ago).  The process was to the final approver step and the request was denied due to no attachment.  I started print screening my attachments when I did them to prove they were there to begin with.  Any help would be greatly appreciated by all in
