INS-32123 Execution of 'GI Upgrade' script failed on nodes: [XXXX01A]

Issue while installing 12cR2 on Win2012R2 on a three node RAC. we have same disk names and disksizes too across the nodes,there are no permissions issues on the folders nor any network related issues. I'm using built in user account as an installing user. anyone has any idea why we are seeing this issue.
I tried multiple times after deinstalling the partial install(stop at 44%) and it reports the same issue,i also see the same issue when using 12201 too.
Exception details
  - PRCZ-2014 : Failed to execute command "C:\Users\svc-gis-oracle\Desktop\V840015-01_grid_new\crs\config\gridconfig.bat" on nodes "XXXX01A"
