Delinquency Fee pending approval but CM Approval flag not set on.

We're testing activation of Credit Manager Approval for Delinquency Fees.
I've set on the flag for this for one delinquency policy but fees for other policy/customers are also going on Pending Approval status.

I've checked that this customer is a stand alone with no parent/child relationship and their policy is not set for CM approval.

Any thoughts on what else could be causing this?
