I got the following IB error: Cannot create a duplicate record, because the combination of Instance

I got the following IB error:  Cannot create a duplicate record, because the combination of Instance, and extended attribute - 25545938  10000 defines a unique record in Installed Base. I followed the instructions on Doc ID 943259.1, but the error did not clear.  Can you provide a next step?  We are running Install Base 11.5.10
