Printed Bill Presentment Invoice - No Logo

I'm trying to change the logo on an existing Bill Presentment invoice. I used the same filename for the new image and copied it to the OA_MEDIA directory. I've run Generate BPA Stylesheet, bounced the server, removed and re-added the logo to the template. No matter what I do, I can see the new logo on the ONLINE preview, but it will not show on the Print Preview. Please help! The following error is in the log file:

[ERROR] Failed to load the image: file:/u04/oraapps/d012appl/apps/apps_st/comn/java/classes/oracle/apps/media/cbc_Geneia.gif
