Opatch error in 12c home

Hi All,

This is shared oracle home,the following error occurs only on prod server, when i query in test server opatch executes fine.

During the upgrade from 11g to 12c have noticed  datapatch is not executed and ignored due to db components are valid,

Need a solution to fix this error,there are some generic solution to detach & attach home, but this is a prod server and there are 11g version running in the server as well, so i'm afraid it might erase all other homes in the server,
