Employee Workflow E-mail (Error : Unable to validate access to Notification)

Morning -

Problem Statement: Employee applies for leave, Manager get  e- mail notification with the following error:Unable to validate access to Notification. No Workflow details attached. When we drill into the workflow itself it is still at Pending Manager approval.


Any idea where to start debugging this error? Has anyone come across this before?

Any guidance would be greatly appreciated.
