Location of custom.xss

In most of our systems, custom.xss is located in $OA_HTML/cabo/styles and works fine.  However, in one system (and other systems subsequently cloned from it), custom.xss only gets applied if it is placed directly into $OA_HTML. 

Something has clearly changed in that rogue environment (perhaps a system profile?), but we have no clue what.  Unfortunately it is complicating our patching processes.  Does anyone have any ideas why this is occurring?
