Error while mounting ACFS volume

Hi,

I created an ASM volume and want to mount it to a mount point.

But when I issue the mkfs command, some error prompt.

Anyone what could be the cause?

[grid@ora08 voldata]$ /sbin/mkfs -t acfs /dev/asm/voldata1-388

mkfs.acfs: version                   = 12.2.0.1.0

mkfs.acfs: CLSU-00100: operating system function: read failed with error data: 5

mkfs.acfs: CLSU-00101: operating system error message: Input/output error

mkfs.acfs: CLSU-00103: error location: ORF_0

mkfs.acfs: ACFS-00526: read of volume disk header failed

mkfs.acfs: ACFS-01004: /dev/asm/voldata1-388 was not formatted.
