Approval sequence not working as per setup

Hi,

We have setup 4 users as approvers. the from amount is 0,1,1 and 100,001.

We have some orders where approval is going to only first 2 users for some orders.

But, now I checked with a new order, it is working correctly.

Could you please help how to track why is is not working correctly for some orders.
