CPIC Annual Update_BY19_Release 4

Hello Everyone!

Attached please find Release 4 of the BY19 CPIC Fast Track along with a document detailing what's new in this package.   This release contains new forms for the Part 3 reports introduced this year as well as updates to the Major IT Business Case Detail forms and some small fixes based on feedback from previous releases.  For example, the "Paperwork Reduction Act Number(s)" section on the MITBC is now a dynamic list. 

The What's New document is separated into 3 sections - rename, modify, and new.   Please review the rename section and make any changes listed before importing the attached package.  Let me know if you have any questions or experience any issues. Thanks!
