After upgrade 12.1.3 to 12.2.6 Benefit Self Service Dependent address not able update

We upgrade system 12.2.6  and after we are not able update dependent address or add address.while entering address after zip code entered (US STYLE) it wipe out address and address field is null out.  Same information add from Personal information via self service we are able to add dependent address

Look like self service page Java controller wipe out form .

Is any one has any suggestion
