Adding Parameters/Checkboxes to Custom Web ADI Template

Hi  WEB ADI Gurus,

   We are in R12.2.5  and have a requirement that the Custom WEB ADI Template needs to have Save and Submit Checkbox just like the buttons on Oracle Applications Page.

Is it possible to have the  check boxes added on the custom WEB ADI Template to achieve this functionality? When the checkbox is clicked it  should pass 'Y' to the Pl/Sql package and 'N' if the checkbox is not clicked. The user don't want to input Y/N as Parameters but want the Save/Submit Checkboxes.

Can you please advise any inputs on this.
