adop phase=prepare fails with OCI-21503: program terminated by fatal error OCI-04030: out of process

We have upgraded our EBS environment from 12.1.3 - 12.2.5( 2 node DB - 2 node App). When we kick off a new patching cycle(adop phase=prepare) we are running into below error.

OCI-21503: program terminated by fatal error

OCI-04030: out of process memory when trying to allocate 1744 bytes (Alloc environm,alloc statement hndl)

remote_execution_result_.xml file recorded the below info

adop exiting with status = 1 (Fail)

.end std out.

Out of memory!

stty: standard input: Inappropriate ioctl for device

Please advice.
