Autopromote failed when releasing Change Order

Hi,

We have a scenario where some Change orders are not getting released and it shows "Autopromotion failed" in the History tab. I have come across cases where if new revision hasn't been set for the Affected items, the release will not get triggered. But here the new revision has been set for the affected item in the CO as well. Why could the autopromotion failed message get triggered and result in the non-release of the CO?
