Reconciliation showing as with preparer when reviewer cliams it

We are using Hyperion 11.1.2.3.501, we are facing a strange issue in ARM where the preparer completed and submitted the reconciliation but when the reviewer claims it, system puts him as a preparer and in stage it is showing as with reviewer level 1.  

1. The reviewer has preparer and reviewer roles.  2. he reviewed reconciliations earlier with out any issues.

Can anyone advise what might be the issue here?
