Form 941 pdf is blank...HRMS RUP 10 not installed yet but 25664473 (quarter 1 statutory) has.

We're EBS 12.1.3 and rdbms 11.2.0.4 on linux and my customer ran the Quarterly tax return* request yesterday and the form 941 pdf generated is 4 blank pages.  I did a little reseach and installed patch  25664473 (Q1 2017 SQWL & JIT STATUTORY UPDATE FOR 121), but this didn't seem to help.  Any ideas?
