Error during Baseline, status remains Rejected

This error occurred during the Baseline of a Budget for a particular Project. in R12.2.5

The subledger accounting validation failed. The cause can be an invalid code combination ID or some invalid data that prevented the system from creating an accounting journal for the event. Please contact your System Administrator to review the debug log

Any idea  why it occurs, what should i took a look at? found this note R12: An Internal Error Has Occurred In The Program xla_je_validation_pkg.budgetary_control. PSA_FUNDS_CHECK_PKG.budgetary_control Failed (Doc ID 1382975.1)
