PO- Missing Action History

Hi All,

1) PO. is inprocess.

2) When clicked on Inquiry> View Action History.

   Gets the error as per the attachment ( missing_action_history)

3) When click on Inquiry> View Approval through Workflow

its shows workflow as "Deferred"

As per the attached document .( Name as "Workflow_doc)

4) Ran the Workflow backgorund process with Defered " Yes", then also the issue is NOT resolved.

5) Document Type for standard PO shows Access Level:"Full"

Please suggest
