Is LogOffDest.swt file part of the installation package?

Whenever we put up another environment, we always encounter a logoff issue where the normal CTRL+SHIFT+X doesn't work. This is usually caused by the LogOffDest.swt file missing from the webtmpl folder. Now I want to ask, is this file part of the installation package? If yes, then why is it always missing?
