OTBI-Not able to fetch Email Type(Home-email) by applying edit formula

Hi

I have requirement to fetch the home-email/Home-Phone/dependent names(father, spouse) along with the primary details and all the details should be in same row. i tried to write the below case statement to filter the details but am not able to fetch those details.

Could some one help me on this formulae.
