Record terminated after reaching terminal status

We have a record that was terminated after it was in terminal status.   This is our pay request, and we do modify the upper form with payment information after the payment is approved. The record writes to our SOV

  The record is showing as terminated after it reached approved status by someone that does not have permission to terminate a pay request to begin with.   We are using 17.6.0.4.  Has anyone had similar issues?
