Charge Account is invalid error while entering project based requisition.

we are getting the below error while entering project based requisition:

"Charge account is invalid. (Program error: An error has occurred while looking up a flexfield segment combination. Please inform your support representative that: The FND_FLEX_SERVER validation function has encountered a null or negative combination ID () while trying to look up the segment values for this combination. This may indicate an invalid entry in the unique ID column of the combinations table.)"

Whatever the account generator is supposed build the code combination, verified that its valid and enabled. Employee level default expense account is valid and enabled. Added Company code segment to GL security rule. Verified that the project has cross-charge enabled so that it can be charged from a different OU.
