12.2.5 patching failed with below errors

ERROR at line 1:

ORA-20001: Parallel Synchronize task failed: FND_INDEX_COLUMNS>, status=8 -1:

ORA-00001: unique constraint (APPLSYS.FND_INDEX_COLUMNS_U2) violated

ORA-06512: at "APPS.FND_INDEX_COLUMNS>", line 66

ORA-04088: error during execution of trigger 'APPS.FND_INDEX_COLUMNS>'

ORA-06512: at "APPS.AD_ZD_SEED", line 1684

ORA-06512: at line 8

I'm running 12.1.0.2 RDBMS with EBS 12.2.3, AD/TXK 8, ATG_PF 6

I have satsified all the ETCC checker requirement.

adop phase=apply patches=19676458,19796559,21270749,21270866 merge=yes patchtop=/scratch/stage/2017/FALL2017/1225

Patch 21270749,21270866 were part of a bug for "An entity definition is required to define values of type WF_WL_LIST_PARAMS
