PO Worklist entries not clearing from worklist

Hi,

We are using delivered Purchase Order workflow in 9.1. Has anyone had an issue with approver worklist entries not getting removed after the user approves a Purchase Order? We only started using delivered workflow last month, and are regularly clearing approved items from users's worklists.
