Shipment issue

Hi,

Our client working on Xe and currently facing one issue. After creating sales order after, successfull ship confirm, shipment number got disappear automatically. Even in F4215 and F4945 there is no record for the order and shipment.

Also in another case one sales order having two shipments but after confirming it shipment number got disappeared.

Can anyone has reported this issue before? Anyone can help me to find out root cause for the same?
