Failed to create Security filters for the following user.

We have a stale user and we removed from all the groups and refreshed security filters but we got the error : Failed to create Security filters for the following user.

We are using latest version 11.1.2.4.006. We don't have this issue in our other DEV environments.

I really appreciate any help.
