Modifier List Qualifiers were added stopped running

I recently updated the List Qualifiers on Modifier and now the Modifier is not being picked up by orders anymore.  I added exclusions to the list qualifiers that already existed.  I added Customer exclusions, but now the modifier does not run for anyone.  I did run the Build Attribute Mappings and QP: Maintains the denormalized data in QP Qualifiers already and that does not seem to help.  Does anyone have any other ideas?
