Unexpected changes on Customer web form (R12.1.3)

Hi everyone!

I found that in OeBS R12.1.3 Customer web form has changed from day to day.

We did not apply any patches. Morover - no user has confessed to change something in persolnalization or to other changes in this form.

Could you help?

Question - why it has changed?

And how we should back to this template from the past (screen1)?
