Employer contribution for medical and FSA

Hello,

Is there any concurrent request available which employee and employer contribution of medical and FSA?  Can someone share query, if possible?  The report looks like

Employee ID   Employee Full name       Payperiod ( or Paydate)      Employee contribution    YTD Employee contribution    Employer contribution   YTD Employer Contribution
