Production Test Upgrade - IP 2013 to IP 2016 (Patch set 14) failed in Import New Customer Repository

We are doing a IP 2013 to IP 2016 upgrade. The Development Upgrep and Upgphys went fine. It is has Binary Sort/Case Sensitive DB.

The Production Test environment,  which is Case Insensitive, Upgrep+Upgphys is encountering a  errors related to Unique Key violations.

2017-09-19 09:47:39 SQL Warning, SQL State 23000, 2601, [Microsoft][SQL Server Native Client 10.0][SQL Server]Cannot insert duplicate key row in object 'dbo.S_APPL_WTMPL_IT' with unique index 'S_APPL_WTMPL_IT_U1'. The duplicate key value is (1-2YKVA-2SFX, executeQuery, <NULL>, <NULL>).
