Failure Codes and Failure Analysis

Hi, I am a newbie to Failure Analysis feature in JDE. I read the Capital Asset Management guide and other articles on Oracle website but none helped me. Can I find any user guide on Failure Analysis.

For example, I read in CAM implementation guide to follow the below steps:

Access the Work With Failure Analysis form.
To setup by code:
1.Select the Failure option and then click Add.
2.Complete the fields to define a failure on the Failure Analysis Revisions form.
3. Select the Failure tab, and complete the Failure Description field.

I entered a failure code and click OK. I see the Failure code is not created - I checked by selecting every Approval option but could not find the record I created.
