blocking_session is not pupulated

Dear Group,

I am using Oracle 12.1.0.2 in three node RAC.

I noticed that our blocking_session in v$session is not pupulated,  reboot the server seems to fix the issue,  but after a while, the problems comes back.

Anybody can help here?
