Segregation of duties in bank account creation

Hi,

What is the process followed in other companies for creation of bank, bank branches and bank accounts.

a) which team creates banks ,bank branches and bank accounts.

b) who assigns them to suppliers and customers

What are the other controls put in place to avoid any internal fraud and enforce segregation of duties.
