Migrate R12.1.3 with 11g RAC to New RHEL 7.x Server with DB upgrade

Hi Friends,

Source : Oracle Apps R12.1.3 with 11.2.0.4 RAC Database on RHEL 5.x (1 App Node + 2 Node RAC)

Target  : New Server Running RHEL 7.x , Oracle Apps R12.1.3 with 12c RAC Database (1 App Node + 2 Node RAC)

How to I migrate from source to Target?

->No upgrade on Apps Tier but on DB from 11g to 12c

->Migrate from RHEL 5 to RHEL 7 on New Server
