12.1.3 PO Workflow completed fine but the PO status didn't refresh

Dear All,

I have PO issues, all my PO's status are INCOMPLETE eventhough in the Action History > Approved by PIC.

I check the:

- Approval Hierachy

- Employee

- Run Fill Employee Hierarchy

- Run PO Analyzer

- I can see in WF Admin that the WF completed.  On the analyse it states for both PO's No WF Item Type found for the combination of Item Type / Item Key

- action history looks complete.  It goes from submit, forward to approved. 

It is like the WF completed fine but the status on the PO just didn't refresh. What should I do?

but the issues still exist.
